Case Management

Case Management services are designed to assist in providing critical HIV/AIDS support services to individuals and their families. This service is supported by Ryan White Title I, Medicaid AIDS Waiver and HUD Funding. It provides services to over 500 individuals.

Housing Services

This program includes both housing assistance and/or rental subsidies. Over 100 people are enrolled in permanent supportive housing or transitional housing programs.

Transportation

Regional transportation is provided to infected and affected individuals throughout Hampton Roads. This program consists of door to door van service, bus tickets, and emergency taxi service. In 2004, services included 8,753 round trip van rides, 6,065 bus tickets, and 455 taxi rides.

Outreach

Outreach services are designed to locate individuals who are infected with HIV and are not currently receiving care. Outreach Specialists work to reduce barriers for these individuals to receive medical care and case management.

Meals Program

Over 9,000 meals are delivered annually to consumers on the Southside and the Peninsula. This program is staffed by volunteers who prepare, package and deliver nutritious meals to support our clients 7 days a week.

Education

HIV/STD prevention education is provided to consumers involved in services with the agency as well as to adults and youth in our communities.

Support Services

Weekly support groups are available for both adults and youth on the Southside and the Peninsula.

Children/Youth Services

We are a licensed daycare provider and support our families with age appropriate activities for children 6 weeks to 12 years of age on the Southside and the Peninsula. Playgroup and Fun Clubs are conducted weekly throughout the year and includes a 6-week summer camp.
